Design a high-end classy yet hipster billboard announcement for best dentist

High end dental office in a spa-like setting. Target audience is a woman ages 35-65.

A billboard size 10 feet 5 inches high by 22 feet 8 inches wide. You are free to be creative but we want a high-end looking design. Think Chanel, Gucci, Tiffany, Vogue Magazine etc. In giant letters it should say "Voted Best Dentist in Napa" . Then the name "Vineyard Dental Spa". A picture of the doctor which is attached. You will need to remove all background from the picture so it is just the doctor. Underneath the picture you will write her name "Dr. Nancy Azizi". That's it, nothing complicated. We are already a very well known office in our town. thus there will be no telephone number, no address and no website on the billboard. Please design it in a way for it to look like an announcement in a high-end fashion magazine.
